Officer or Staff Misconduct Record
MG06B – Officer or Staff Misconduct Record is used by an official witness to tell the CPS about relevant criminal convictions, cautions, misconduct outcomes or adverse judicial findings. It gives the CPS enough information to decide whether any disclosure is required.

Additional guidance
MG06B is part of the prosecution disclosure process. It is used where a police officer, member of staff or other official witness in a case has information that may be relevant to their credibility or to disclosure.
The form may record:
- criminal convictions, cautions or penalty notices for recordable offences;
- misconduct findings or outcomes;
- adverse judicial findings;
- planned misconduct or related hearings; and
- later changes that may need to be reported to the CPS.
The individual official witness is normally responsible for completing the disclosure information, with assistance from the Professional Standards Department where needed. The form must contain enough detail for the CPS to make an informed decision about whether the information should be disclosed.
MG06B is not an unused material schedule and does not decide whether the information will be disclosed to the defence. It is a record provided to the CPS, which makes and records the disclosure decision under the applicable legal and procedural framework.
