Summary
we found that outcomes for prisoners remained not sufficiently good in safety and respect, and poor in purposeful activity. A shortfall of staff across different grades and departments was affecting outcomes for prisoners. Retention of staff was also poor. Violence was too high and prisoners at risk of suicide and self-harm were not sufficiently well cared for. Partnership and collaboration arrangements between the prison and the health care provider needed to improve. Living conditions were not good enough and time out of cell (see Glossary) for prisoners was inadequate.
